Each new patient is assessed by a developmental physician and dentist, who then determine the necessary components for their diagnostic and therapeutic plan. To start the admission process, complete our new patient application form, which goes to  our secure, HIPAA-compliant platform.

Afterward, the Kramer Davis team will contact the patient/guardian to confirm acceptance and guide them through the complete patient intake process.

To start the admission process, the referring clinician fills out a referral form, which is sent to a secure, HIPAA-compliant platform.
