STEP 1
At Kramer Davis, we're committed to delivering compassionate, interdisciplinary care to those with intellectual and developmental disabilities. To ensure we can offer comprehensive services, please download the patient packet below and complete it for initial and ongoing care, updating your health information annually.
STEP 2
To provide you with the best care, we require information from any other entities involved in your health care. Please fill out both of the following Release of Information (ROI) documents to allow us to request your protected health information. Each document ensures we have your consent to gather necessary health records and other information critical to your care at Kramer Davis.

STEP 3
Ensure the patient packet and Release of Information documents are fully completed and signed as required. Please upload the patient packet promptly along with all requested documents.
